  • No desire to continue dialysis treatment.

  • Multiple ER visits and/or hospitalizations in past 6 months

  • Shortness of breath/chest pain at rest requiring continuous O2

  • Severe dementia or advanced neurological diseases that result in incontinence, inability to ambulate without assistance and/or inability to communicate.
