We are looking for a compassionate and skilled Occupational Therapist (OT) to join our team and help our patients improve their ability to perform daily living activities and achieve greater independence.
Responsibilities:
Evaluate and assess patients' physical and functional abilities.
Develop individualized treatment plans to address patients' specific needs and goals.
Implement therapeutic interventions to enhance patients' daily living skills, motor functions, and cognitive abilities.
Educate patients and their families on techniques and strategies to improve their daily living and quality of life.
Monitor and document patient progress, adjusting treatment plans as necessary.
Collaborate with other healthcare professionals to provide comprehensive care.
Maintain accurate and timely records in compliance with healthcare regulations.
Participate in team meetings and case conferences as needed.
Qualifications:
Master's or Doctoral degree in Occupational Therapy from an accredited program.
Valid state licensure as an Occupational Therapist.
Previous experience in home health care is a plus.
Excellent communication and interpersonal skills.
Strong organizational and time-management abilities.
Ability to work independently and as part of a multidisciplinary team.

Responsibilities:
-Conducting comprehensive assessments of patients with communication and swallowing disorders
-Developing and implementing individualized treatment plans
-Providing therapy services to improve speech, language, cognitive-communication, and swallowing abilities
-Collaborating with interdisciplinary team members to ensure holistic care for patients
-Documenting patient progress and maintaining accurate records
Requirements:
-Master's degree in Speech-Language Pathology from an accredited program
-Valid state licensure as a Speech-Language Pathologist
-Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
-Previous experience in a healthcare setting, preferably in home health.

Home health experience is preferred, but not required, and at least six months experience is preferred, but not required. 1st Care At Home is seeking a Physical Therapist (DPT) to join in team in Eastern service area of Virginia! PTs are responsible for the delivery of skilled treatment to homebound patients, as prescribed by a physician, and for the purpose of restoration and rehabilitation; works with patient/family in instructive capacity in preventative and maintenance measures; functions as a member of the home care team. This position can be full-time (with 4-5 day work schedule) or PRN.
